Presentation: Creating a culture of quality improvement within your organization

Rachel Edwards is a quality professional with over a decade of experience in healthcare and seven years of experience transforming health data into actionable insights. Currently, she is the Quality Analyst at Church Health, where she specializes in data visualization, performance measurement, and project management to support patient-centered care initiatives.
At Church Health, Rachel has led several cross-departmental projects focused on improving patient care, including increasing hypertension control rates, creating and streamlining a patient feedback system, redesigning the patient registration process, and implementing a new population health software. Her work includes developing dashboards and other reporting tools used by senior leadership and clinical teams to track metrics and implement data-informed interventions. She also teaches a quality improvement class, open to all staff and interested external organizations, where participants can learn about the basics of QI and practice leading their own project.
Rachel holds a B.S. in Business Management and an M.B.A. in Healthcare Administration from Concordia University Wisconsin. She is also an active quality improvement mentor and speaker, passionate about using data and methodologies to advance patient-centered care and healthcare equity.

Madeline Potter is the Clinical Quality Improvement Specialist at Church Health. In her role, she supports quality improvement efforts across the organization and identifies opportunities to promote patient-focused, quality care through data collection, analysis, interpretation, and reporting.
Madeline is passionate about improving population-specific outcomes through bridging gaps in patient care. This is accomplished by monitoring metrics, investigating clinical data, and implementing quality improvement tools and methodologies in collaboration with other key roles and departments. Additionally, she co-facilitates the Quality Improvement course, where she coaches participants in their projects and promotes a culture of continuous improvement.
Madeline holds a B.S. in Health Sciences from the University of Memphis, where she studied quality improvement methodologies, project management, program evaluation, and epidemiology. She plans to pursue a Master’s in Public Health with a concentration in Epidemiology to further enhance community health outcomes through data-driven strategies.
